Community Nursing Care

Registered Nurse-led clinical care at home — for participants with complex health needs.

Clinical Care That Lets You Be a Family Again

When a loved one’s health becomes complex, the weight of responsibility can feel overwhelming. You shouldn’t have to be both a family member and a full-time nurse. At Life Reach, we step in so you can go back to being a daughter, a son, or a partner.

Our Registered Nurses bring clinical care directly to your living room — so your loved one is supported by qualified clinicians, and your family doesn’t have to carry the clinical load alone.

Why Families Trust Life Reach

Many NDIS providers focus on social support and personal care. Life Reach is built around clinical care — directed by Registered Nurses with hospital and community nursing backgrounds.

We understand the anxiety families feel after a hospital discharge. We speak the same language as your doctors, but we speak to you with the warmth and clarity you deserve. Our nurses watch for the early signs that matter — and act on them — so small problems don’t become emergencies.

Why us?

Who is it for?

This service is built for NDIS participants with unstable or complex health profiles. We work with:

What We Do

Our Registered Nurses provide the clinical care that sits beyond the scope of a standard support worker — the tasks that legally and safely require a registered clinician.

Advanced Infection Control

Strict aseptic techniques and hygiene protocols for participants with compromised immune systems — particularly important after surgery or for participants with central lines, PEG sites, or open wounds.

Comprehensive Clinical Assessments

Head-to-toe nursing assessments including vital signs (blood pressure, oxygen saturation, heart rate), respiratory checks, and neurological observations.

Medication Governance

Administration of complex medications, including subcutaneous injections (insulin, Clexane / enoxaparin), Schedule 8 medication oversight, and IV management where required.

Complex Wound Management

Assessment and dressing of surgical incisions, pressure injuries, and ulcers — using current Australian wound care guidelines.

Coordination With Your Doctors

Our Registered Nurses report back to your GPs and treating specialists, so the clinical picture stays coherent across your medical team.

Family Education and Worker Training

We train family members and your daily support workers to recognise the early signs of clinical deterioration — so everyone in your participant's circle knows what to watch for, and what to do.

24/7 Support

Ready to reach for a higher standard of care?

Whether you are a family member looking for support or a professional seeking a reliable clinical partner, the Life Reach team is here to help.
