High-Intensity Care

Nurse-led support for participants with complex clinical needs — for the tasks that have to be done right, every time.

Clinical Care for the Tasks That Can't Be Delegated to Just Anyone

When daily living needs more than a helping hand, Life Reach provides the clinical support to manage complex needs safely at home. High-intensity care isn’t just “doing a task” — it’s about doing it correctly, every shift, with a Registered Nurse close enough to step in if something changes.

We bring the rigour you’d expect from a clinical setting, into the comfort of your own home.

Why Life Reach for High-Intensity Care

In the NDIS sector, “high-intensity” is sometimes treated as just a label. At Life Reach, it’s the work we’re built around. We started this agency because we saw too many participants with complex needs being supported by workers who hadn’t been properly trained or supervised for the clinical tasks they were performing.

We don’t just assign a worker to a high-intensity shift. Every team member is personally trained and competency-checked by our Clinical Directors, Sisam and Sharmila, for the specific clinical tasks they’ll perform. The same rigour you’d expect on a hospital ward — applied to the participant’s own home.

Why us?

Who is it for?

This service is built for participants whose NDIS plans include “high-intensity” funding or descriptors. We support:

Participants with Invasive Supports

People requiring enteral feeding (PEG/PEJ/NG), catheters (IDC/SPC/ISC), or stoma management.

What We Do: The High-Intensity Scope

Our team performs the critical daily tasks that require specialised knowledge and strict adherence to clinical protocols. Our scope includes:

Enteral Feeding (PEG, PEJ, NG)

Administration of nutrition and hydration via feeding tubes, with site care, equipment troubleshooting, and clinical observation for tolerance and complications.

Stoma & Colostomy Care

Skin integrity management and pouching system support. Our Registered Nurses monitor stoma health and review the care plan when complications arise.

Complex Bowel & Catheter Care

Management of indwelling (IDC), suprapubic (SPC), and intermittent (ISC) catheters, alongside complex bowel regimes prescribed by the participant's medical team.

Tracheostomy & Respiratory Support

Airway management, suctioning, and nebuliser therapy for participants with compromised respiratory function.

Infection Control & Aseptic Technique

Strict aseptic technique applied to every high-intensity task — particularly important for catheter, PEG site, stoma, and wound care, where infection risk is highest.

Wound Management & Pressure Care

Skin assessments and dressing changes for complex wounds and participants at risk of pressure injuries, using clinical positioning and current Australian wound care guidelines.

24/7 Support

Ready to reach for a higher standard of care?

Whether you are a family member looking for support or a professional seeking a reliable clinical partner, the Life Reach team is here to help.

FAQs

High-Intensity Support FAQs

Understand how we manage complex clinical needs with our specialised and highly skilled support services.

1. What makes a support "high-intensity"?
Under NDIS, “high-intensity” describes supports that involve a higher level of risk or require trained clinical skills — for example, feeding tubes, catheters, complex seizure management, or tracheostomy care.
Yes. Each support worker is delegated and competency-checked by our Registered Nurses for the specific clinical tasks they’ll perform on a participant’s plan — and only those tasks. They’re not asked to perform tasks outside their competency sign-off.
We follow current clinical protocols for tube flushing, site cleaning, and pre-feed checks (including aspirate checks where indicated). Our Registered Nurses review the process regularly so the participant’s nutrition is delivered safely.
We provide an after-hours clinical support line for current participants. If a worker or family member is concerned, they can speak directly with one of our Registered Nurses for guidance.
Our staff use aseptic technique for catheter care — the standard approach for reducing infection risk. We monitor for early signs of infection and act early if anything changes.
Yes. Our team supports stoma care including pouching system management and skin integrity around the stoma site.
Yes. We build and manage 24/7 rosters for participants who require continuous clinical monitoring, including active overnight nursing support where clinically required.
Yes. Our Registered Nurses provide formal competency sign-offs that meet NDIS Practice Standards — so anyone involved in clinical care is trained, signed off, and clear on what they can and can’t do.
High-Intensity Care is typically funded under Core Supports — Assistance with Daily Life, at the high-intensity rate for participants who need clinical-level personal activities. If you’re not sure how this maps to your plan, we can review it with you.
