Choosing a SIL home is about more than a roof over your head — it’s about finding a place where the daily rhythm, the support team, and the clinical care all fit together. For participants with complex clinical needs, that fit is hard to find. Most SIL settings are built around general support, not the clinical work that complex needs actually require.
Life Reach SIL is different. Built and led by Registered Nurses, our SIL placements are designed around the participant’s clinical needs first — so the home, the staffing, and the day-to-day care all work together, not against each other.
Participants with complex physical or neurological needs who require Registered Nurse-led protocols and a clinically trained support team to live safely outside of a hospital or institutional setting.
We work as an open, communicative partner — providing families with regular updates, structured reporting, and direct access to our clinical team when something needs attention.
Conditions including Acquired Brain Injury (ABI), tetraplegia, spinal cord injury, and progressive neurological conditions like MS, MND, and Parkinson's — all of which need the right equipment, the right handling, and the right clinical oversight.
For participants moving from a hospital ward or rehabilitation unit into permanent SIL accommodation. We work alongside discharge teams to coordinate the transition so it happens without gaps in clinical care.
Our SIL placements are based in metropolitan Melbourne and Perth — close to the hospitals, treating specialists, and allied health teams participants and families rely on.
For participants with complex clinical needs, the daily quality of care directly affects health, safety, and the kind of life that’s possible at home. Most SIL is built around personal care and supervision. Life Reach SIL is built around clinical care, with personal support and supervision wrapped around it.
Because we’re nurse-led, our Clinical Directors — Sisam and Sharmila — make sure every SIL placement runs to current clinical protocols. We don’t just “staff a house.” We manage medication safety, monitor nutrition and hydration, maintain skin integrity, and watch for the early clinical changes that, if missed, end up sending participants to hospital.
Support with all activities of daily living — including dressing, grooming, hygiene, and toileting — delivered with respect for privacy and dignity.
Regular Registered Nurse visits to review care plans, monitor clinical observations, and check that high-intensity tasks (such as PEG feeding, catheter care, or ventilation support) are being performed correctly.
Support with grocery shopping, meal preparation, and dietary requirements — including modified-texture diets, enteral feeding regimens, and culturally important food preferences.
Support to access the wider Melbourne and Perth communities — attending appointments, social activities, family connections, and the things that make a house feel like a life, not just a placement.
Medication administration following prescribed charts, with Registered Nurse audits and clinical observation for side effects or changes that need to be reported back to treating doctors.
Daily collaboration with the participant's OTs, physios, speech pathologists, and behaviour support practitioners — implementing therapy goals at home and feeding clinical observations back to the wider team.

Whether you are a family member looking for support or a professional seeking a reliable clinical partner, the Life Reach team is here to help.
